In Staten Island, a healthcare revolution is underway. Dr. Vladimir Barayev, a board-certified pediatrician specializing in autism, is on the forefront of this movement, championing a new way of delivering care through the Direct Primary Care (DPC) model. In Episode 161 of the My DPC Story Podcast, Dr. Barayev discusses his journey into the world of DPC and how it’s redefining the patient-doctor relationship.

Building Strong Foundations With DPC

Direct Primary Care (DPC) is healthcare stripped down to its most fundamental elements — a patient, a doctor, and quality time together. This model eliminates the complexities and constraints of insurance-based practices, replacing them with a simple, subscription-like service where patients pay a monthly membership fee for their care.

Dr. Barayev’s approach to DPC pediatric care is particularly unique. He operates a practice that shuns waiting rooms and operates on an ‘one patient at a time’ philosophy, with appointments that last an hour and includes a playroom to ease the anxiety often experienced by children in healthcare settings. Staten Island has embraced Dr. Barayev for his commitment to high-quality, personalized healthcare for children, especially those with special needs.

Connecting With The Community

Community involvement is at the heart of Dr. Barayev’s success. He leverages his relationships with local organizations, such as the Grace Foundation, to not only spread awareness about his services but also demonstrate his dedication to the well-being of Staten Island’s children. His practice is also the first of its kind across all five boroughs of New York City, setting a precedent for healthcare innovation.

Dr. Barayev emphasizes that understanding community needs is key. His decision to focus on autism evaluations stemmed from a genuine desire to meet a pressing need in his area, showing the power of listening to and engaging with one’s community.

Harnessing The Power Of Social Media and Networking

Dr. Barayev has turned to digital tools and platforms, including TikTok, Instagram, Facebook, LinkedIn, and YouTube, to further propel his practice. These serve as marketing channels and allow him to educate and connect with families beyond consultations. His digital presence reinforces his role as a physician and an accessible resource for families seeking guidance.

Through the effective use of Google reviews and Facebook groups, Dr. Barayev has crafted a marketing approach that resonates with modern-day patients, creating a strong network that drives his practice forward.

Challenging The Status Quo

Throughout the podcast, Dr. Barayev frequently addresses the limitations of traditional primary care settings and how the DPC model offers a viable alternative. He discusses the concept of ‘moral injury,’ where physicians feel caught between their professional obligations and their desire to provide individualized, quality care. DPC, he argues, resolves this conflict by centering the doctor-patient relationship without the interference of insurance companies.

The Impact Of Entrepreneurship And Mentorship

No journey is without its influencers, and Dr. Barayev credits much of his business acumen and motivation to his mentor, Dr. Nitin Gupta. His mentorship and resources like the My DPC Story Podcast have provided invaluable insights that have shaped Dr. Barayev’s entrepreneurial spirit.

Dr. Barayev candidly shares the challenges and rewards of running a DPC practice. Statistically, medical businesses have high failure rates, but Dr. Barayev’s story is one of perseverance and adaptability, proving success is possible with the right mindset and the determination to meet patient needs.

Shaping The Future Of Healthcare

Dr. Vladimir Barayev’s dedication to pediatrics and the DPC model is more than just a career choice — it’s a commitment to shaping children’s future health through education, lifestyle advocacy, and personalized patient support. His entrepreneurial spirit reflects the evolving healthcare landscape and sets a strong example for current and future DPC practitioners.

By Maryal Concepcion, MD

A Sacramento native, Dr. Maryal Concepcion is a Filipinx creator and connector. She attended the University of California at Davis where she graduated in 2005 with a double major in Anthropology (BS) and Evolution & Ecology (BA). She had an unforgettable time as a California transplant when she got her MD at Creighton University in Omaha, NE. It was there she discovered hayrack rides, found her Husband, Dr. Jeremiah Fillo, and discovered her love of rural family medicine. Her residency training was in the unopposed family medicine program at Doctors Medical Center, Modesto, a training extension through the UC Davis Medical School. During her time in training, she returned to rural Superior Nebraska to complete proficiency training in performing colonoscopies as part of her rural family medicine concentration. After residency, she moved to Arnold, CA where she worked under the fee for service model with her Husband doing full-scope care for over five years. Though grateful for her experience in this role, she was driven to seek a better way of life and a desire to practice insurance-free medicine and that was when she discovered DPC. She is the founder and host of the podcast My DPC Story, a weekly podcast that educates and inspires by featuring DPC and direct care physicians. Find it at mydpcstory.com and on all major podcasting platforms. Most recently, she became the CEO and wearer-of-all-the-other-hats at Big Trees MD, the first Direct Primary Care in Arnold, a rural town of 4,000 in the Stanislaus National Forest located in Northern California. Her clinic was created as a telemedicine and home visit model without a brick and mortar space. But, the title she is most proud of is being a Mama to Asher & Nolan, her two ridiculously awesome and cuddly boys!
